A US military inquiry has concluded that failure by US troops in Afghanistan to follow rules of engagement resulted in the deaths of dozens of civilians.
Pentagon officials investigated seven air strikes against Taliban militants carried out in Farah province on 4 May and found that three of them did not comply with military guidelines. The report says the strikes killed at least 26 civilians but admits that the real figure could be much higher. President Hamid Karzai insists 140 civilians were killed in the strikes.
